The tool kit you will find at the link below was created by the Empire Justice Center with funding from the New York State Health Foundation.  The tool kit is designed to guide community health center staff in their efforts to help patients access public health insurance or other kind of help paying for medical care.  The toolkit was last updated in August of 2009.

Each topic contains fact sheets of 3-4 pages  with charts and/or sample forms following where applicable.  Each fact sheet represents either one of New York’s basic public health programs or an important tool you can use to help your patients qualify for a public health programs.
